Getting Your Safety Eyewear
Step 1: Obtain an ACI Form
Your Eyesafe ACI Form has been tailored to MSA's requirements. If your company has special requirements, reach out to us and we will design an ACI form specific to you. Ensure your ACI Form has the name and phone number of the appropriate authorized safety officer or human resources personnel.

Step 3: Choose Your Eyewear
The trained staff at your optometrist's office will follow the specific criteria set up for your company to ensure you are provided with the safety eyewear you require. Once you have received your safety eyewear, you may return at any time for adjustments at no charge.
Step 2: Book an appointment with your optometrist for an eye exam. 
Present your ACI Form upon arrival at your optometrist’s office. A list of participating Saskatchewan optometrists can be found at eyesafesk.ca and searched by name or location. Look for the Eyesafe logo to find a participating optometrist.
