M4A's Home & Community Based Services (HCBS) Program

The Elderly and Disabled (E & D) Medicaid Waiver program is designed to allow elderly and/or disabled individuals who would otherwise require care in an institutional setting (nursing home) to remain at home. M4A provides the long-term care support needed for our clients to remain in their local communities.
Available services include case management, homemaker services, personal care, and respite care. Individuals applying for Medicaid Waiver services must meet certain medical criteria and have resources below a certain limit. Waiver programs include Personal Choices, ACT, and the 530 Program.

A MEDICAID WAIVER CLIENT'S STORY

One of our Medicaid Waiver clients lives in a small apartment, alone and is homebound, with no family support. He has multiple chronic medical conditions, leaving him extremely frail and unable to complete daily tasks like cooking a meal, taking a bath or cleaning his home. Since being admitted to our program, he now has an advocate and also a caregiver who provides personal care, homemaking services, runs errands, and provides companionship. He also has a Case Manager who continuously checks on him, provides additional resources as needed and reassures him that people are there for him and care about him.

The VA has a number of services designed specifically to support Family Caregivers. They have several support and service options designed with caregivers in mind. The programs are available both in and out of your home to help you care for the Veteran you love and for yourself.
