PART TIME COMMUNITY OMBUDSMAN (OMBUDSMAN REPRESENTATIVE)

Location: Central Region (Blount, Chilton, Shelby, St Clair and Walker counties)
Compensation: Receive travel reimbursement for work-related mileage and paid on-the-job training provided.
 
Job Description:
  • Regularly visits nursing home and assisted living facilities to develop and maintain relationships with residents, ensures communities are complying with Residents’ Rights and promoting quality of life, and addresses any issues reported during the visit;
  • Investigates and resolves complaints in a timely manner. Complaints may come from family members, friends, staff, or residents themselves. Complaints may be related to problems with staff or care received, family conflicts, or resident-to-resident issues;
  • Maintains good rapport and cooperative relationships with long-term care communities to provide education for staff and conduct investigations;
  • Collaborates with other agencies such as the Alabama Department of Public Health, Department of Human Resources, and the organization’s Legal Services Provider; 
  • Manages difficult or emotional customer situations using best customer service skills;
  • Fully meets all commitments and assignments;
  • Recruits and trains volunteers to increase the exposure of the ombudsman program in long-term care facilities. The ombudsman also maintains support for and regular contact with volunteers;
  • Advocates for culture change and person-centered care in long-term care communities;
  • Provides information and counseling on topics related to long-term care, including but not limited to, applying for long-term Medicaid, long-term care insurance, pay status in nursing homes and assisted living facilities, and what to expect upon moving into long-term care;
  • Promotes advocacy and awareness of the ombudsman program through family councils, resident councils, health fairs, staff in-services, written materials, and other speaking engagements within the organization’s service area;
  • Monitors changes in policy and legislative activities to stay abreast of current trends and how they affect long-term care;
  • Serves to support other grants as needed/ other duties as assigned.
Education/Experience:
Preferred Bachelor’s Degree in Social Work, Psychology, Social Science, Public Health or Public Administration.  Experience with long-term care and caregivers preferred. Experience with public benefits and programs, and compliance preferred. Experience with long-term care and caregivers required.

PART TIME - REMOTE RN SUPERVISOR - CARE TEAM LEAD

Location: Central Region (Blount, Chilton, Shelby, St Clair and Walker counties)
Compensation: Receive travel reimbursement for work-related mileage and paid on-the-job training provided.
 
Objective: Serve the Medicaid eligible clients who would otherwise require nursing home care and are at risk for nursing home placement.  The goal of the Medicaid Waiver (MW) Program is to keep clients in their own homes and to delay/avoid institutionalization by locating, coordinating and monitoring services.

Job Summary: The Medicaid Waiver Service Program RN Supervisor, in partnership with the Lead Case Manager, supervises Team Leaders and all Medicaid Waiver Case Managers. This position reports directly to and assists the DOP in overseeing all aspects of the MWS program, including, but not limited to, quality assurance, compliance, successful transition to and implementation of Medicaid Managed Care (under the INCs or Integrated Care Networks), FAMCare, and Compliance Bridge.

N4A's New Health Information Technology Learning Collaborative: Call for Applications

The Aging and Disability Business Institute is announcing a call for applications for community-based organizations (CBOs) to participate in a new learning collaborative that will assist aging and disability CBOs in making strategic decisions around investing in health information technology (HIT) systems to collect, manage, and analyze service data while enhancing program quality, client satisfaction, and service reimbursement.
The Health Information Technology Learning Collaborative (HITLC) will provide up to 12 CBOs with targeted technical assistance and a platform to share their learnings. The key outcomes of participation in the HITLC will be the successful implementation and use of administrative systems to grow the capacity and impact of participating CBOs, and the development of tools that can assist the larger field of CBOs with the decision-making process.
