Tip of the Month
Have a safe-discharge checklist!
As physicians we are well programmed, so to speak, to ask the patients "What brought you to the ED?". The answer to which is effortlessly obtained during the first few seconds of patient encounter. A more basic question, should be "What brought you out of the ED?" It might seem unusual question but it is so because, this is a question not to be asked, but to be answered. Here where safe-discharge checklist comes in to play. The safe-discharge checklist is a tool developed to facilitate a seamless transition from inpatient care to home. A set of questions\criteria that engages patients and their family in the disposition plan and ensures efficient disposition . Plenty are the available hospital discharge templates. Customize your own checklist, discharge your patients safely, and limit avoidable ED revisits!
