Discharge Planning Meeting
A discharge planning meeting takes place before a person is discharged from
hospital. It is a meeting with hospital staff and the person being discharged. The person being discharged may want to invite friends, family or another support person or professional.
It looks at what you are going to do after you leave hospital.
What supports are in place?
Have you prepared to go home?
Sometimes people who live alone will take a day trip home to check that everything is in order before they get discharged.
When I worked in community mental health I encouraged clients that if
there was any sign of discharge make sure they request a discharge planning meeting.
I would ask them who they wanted at that meeting and let them know they can invite
people and they have a right to one.
I would ask them if they want me to attend the meeting. If they wanted me to attend I would regularly remind nurses to invited me to the discharge planning meeting.
One day I was rung and told that a client was getting discharged in half an hour.
I asked "When was the discharge planning meeting?". They said there wasn't one.
So I said "Let me know when the meeting is because the client wants me to attend".
The hospital rescheduled discharge until after this meeting had occurred.
